What should I take to my first appointment?

I want to switch to Nittany Valley Medical Associates for my health care. What do I do first?
Please start by visiting the health office in your facility to obtain the necessary paperwork to begin the process. You will be asked to fill out a basic demographics form and an Authorization to Release Medical Records form. This second form will then be sent to your current physician requesting the release of your records. Please understand that until we receive your records you are still under the care of your current physician. You will not be a patient of Nittany Valley Medical Associates until you have your initial appointment with us.

How long does it take for me to get in to see the doctor for my first visit?
It is likely that we are able to schedule your initial visit within a week of receiving your records. Please call our office for current appointment availability.

What should I do if I need a refill?
If your medication is completely out of refills please call our office and your doctor will send a new prescription to your preferred pharmacy. If there are still refills left on your bottle please call your pharmacy directly to have it refilled.

How long does it take to get a medication filled?
We ask that you allow at least 24 hours for the prescription to be filled. It is recommended that you call approximately a week before you run out of the medication to allow adequate time to have it filled.

Where is your office located?
Our providers are the resident physicians at The Village at Penn State, Foxdale Village, The Oaks at Pleasant Gap, Westminster Woods at Huntingdon, Valley View Retirement Community and Harmony at State College. We are not currently accepting patients from the general public.
