Coordinated Health Care

Nittany Valley Medical Associates is working to become a recognized Patient Centered Medical Home through the National Committee of Quality Assurance (NCQA). This program sets high standards for medical offices to qualify. The importance of this recognition is to ensure providers are coordinating with patients and their families to reach an informed decision in every aspect of the patients’ health care.

Obtaining Advice

Our office hours are 8am to 4:30pm. Any calls or messages received during that time will be directed to the provider as necessary. Any calls after hours will be forwarded to our answering service who will reach out to the provider on call. All calls will be returned within 1 hour. Any calls requesting a future action will be returned the next business day.


Acute Care

Our providers want to bring the best possible care to our patients including appointments before/after normal business hours if needed.


Medical History

In order to provide the best possible care to our patients we ask that they provide us with a complete medical history. Cooperation in this aspect of health care is crucial to reach an educated decision as a team. We provide patients with a new patient form that streamlines the basics of this process. We still require full medical records from previous providers.


Behavioral Health

Our practice provides services to meet all of your primary care needs. For any behavioral health concerns we direct your care to a local specialist who we feel is the best fit for the patient and their needs.


Equal Access

We strive to make the most out of every patient’s visit with us. This includes becoming credentialed with as many insurance companies as possible to save the patient time and money. Please feel free to contact our billing office at (814) 272-5805 to ensure that we accept your insurance.

We offer payment plans for any outstanding balances due to deductibles, large co-payments, and coinsurance.


Uninsured Patients

We are aware that some patients may not have insurance but we are more than experienced to assist patients with finding an insurance that we accept. We are equipped to provide patients with informative phone numbers and websites so that they can research multiple insurance companies to find the absolute best fit for them.


Record Transfer

Our practice has a form we ask patients to fill out so that we can obtain their records from previous physicians. It is possible that the patient may have to fill out one form per physician/specialist. The main contact for this form, and who will retrieve the records, is the nurse that is coordinated with the primary care physician of their choice. Coordinating Care

Our practice works with referring physicians to ensure that all records and information is given to them. We work to make sure that information is received from outside providers so that our physicians are actively involved in patient care.
