Our office hours are 8am to 4:30pm. Any calls or messages received during that time will be directed to the provider as necessary. Any calls after hours will be forwarded to our answering service who will reach out to the provider on call. All calls will be returned within 1 hour. Any calls requesting a future action will be returned the next business day.
Our providers want to bring the best possible care to our patients including appointments before/after normal business hours if needed.
In order to provide the best possible care to our patients we ask that they provide us with a complete medical history. We provide patients with a new patient form that streamlines the basics of this process. We still require full medical records from previous providers.
Our practice provides services to meet all of your primary care needs. Behavioral health concerns will be directed to a local specialist who we feel is the best fit for the patient and their needs.
We strive to make the most out of every patient’s visit with us. This includes becoming credentialed with as many insurance companies as possible to save the patient time and money. Please feel free to contact our billing office at (814) 272-5805 to ensure that we accept your insurance.
We offer payment plans for any outstanding balances due to large deductibles, co-payments, and coinsurance.
All new patients will be required to complete a formal request for medical records for all previous physicians. Please contact your facility to obtain the appropriate documentation or call our nursing staff at 814.574.7756.
Our practice works with referring physicians to ensure that all records and information is given to them. We work to make sure that information is received from outside providers so that our physicians are actively involved in patient care.
